Q1: Can I get somebody to help me complete the electronic Personal Questionnaire?

Yes, you can get another person to initiate and partially complete the electronic Personal Questionnaire but as the individual to whom the information relates you will need to check and submit the completed form using the Commission Portal.

As a result you must at some point register and create a profile within the Commission Portal.

Q2: If I need to partially complete an electronic Personal Questionnaire on behalf of an applicant Principal Person or Key Person, should I log on using their registration details?

No, you will need to register and log in to the Commission Portal using your own details.

Once you have completed the elements of the electronic Personal Questionnaire for which you have information, the partially completed Personal Questionnaire should be transferred to the applicant for finalisation and submission by them.

Q3: If I partially complete an electronic Personal Questionnaire on behalf of an applicant Principal Person or Key Person can I complete the review stage before transfer?

Yes, the electronic Personal Questionnaire can be reviewed at any stage of completion.

The review function is both a safety measure to confirm that all mandatory fields have been completed and also provides the applicant with a consolidated view of the Personal Questionnaire information prior to submission. As such it was designed with the intention that it would be utilised by an applicant immediately prior to submission of their information to the Commission.

Q4: Once a partially completed electronic Personal Questionnaire has been transferred to me as the applicant Principal Person or Key Person do I need to complete and submit it immediately?

No, the electronic Personal Questionnaire will be available from the “My Forms” section of the Commission Portal indefinitely. If you attempt to sign out of the Commission Portal without saving the system will prompt you to save your work.

Q5: If I partially complete an electronic Personal Questionnaire on behalf of an applicant Principal Person or Key Person will I be advised when the Personal Questionnaire is submitted?

You will not receive an automatic email notification when the electronic Personal Questionnaire is submitted. However, all electronic Personal Questionnaires that you originate, including those that you transfer to an applicant, will be listed on the “My Forms” section of the Commission Portal. When an applicant submits their electronic Personal Questionnaire as originator your list will be updated with the date and time of submission.

Q6: If I partially complete an electronic Personal Questionnaire on behalf of an applicant Principal Person or Key Person, following submission of the completed form will I be able to see the final document?

Each electronic Personal Questionnaire is linked to a single email address therefore once it has been transferred you will not be able to access the document – as originator of the Personal Questionnaire you will be able to see the document in the “My Forms” section of the Commission Portal, but you will not be able to open the document and access the information.

Q7: If I partially complete an electronic Personal Questionnaire on behalf of an applicant Principal Person or Key Person will I be advised when they make future changes (i.e. add new appointments)?

Following submission of the initial electronic Personal Questionnaire, if a Principal Person or Key Person updates their information and submits a new form to the Commission you will not be advised unless your registration email address is provided to the Commission through completion of 12.20 of the electronic Personal Questionnaire.

Q8: Can I assist the Principal Person or Key Person with subsequent changes to their Personal Questionnaire data?

It depends.

If the Principal Person or Key Person wishes to update their personal details then you will not be able to assist as these details
should be amended through the “My Profile” section of the Commission Portal.

However, if the update involves applying for another or alternative positions then you may be able to assist. In this scenario the Principal Person or Key Person will need to create a copy of the latest submission and transfer the copy document to you.
Following application of the updates you will need to return the document to them for submission.
